The Sierra Madre Paramedic Subscription Program, offered by the City of Sierra Madre, helps reduce the cost of emergency medical responses. This annual membership is available to Sierra Madre residents as an alternative to paying higher direct fees for paramedic and ambulance services. A single emergency paramedic call can exceed $3,000, and many insurance companies don’t cover the full amount. As a member of the Paramedic Subscription Program, your subscription acts like secondary insurance, with no additional out-of-pocket costs to you.
Program Features
- Subscribers must be a resident of the City of Sierra Madre.
- If you have insurance, your enrollment fee covers any deductible or co-payment.
- Membership is valid for one year from date of payment.
Enrolling in the Paramedic Subscription Program
1. Complete the Enrollment Form
2. Include a check or money order for $89 per subscriber, please make checks payable to the City of Sierra Madre.
3. Mail your application and payment to:

Subscribers must be a resident of the City of Sierra Madre. Payment of $89 per individual subscriber is due upfront. Subscriptions are valid for one year from date of payment and renewal notices will be mailed one month prior to expiration. Primary and Secondary insurance providers will be billed prior to payment by the City of Sierra Madre. Fee protection only applies to emergency medical treatment and/or ambulance treatment rendered by the Sierra Madre Fire Department. Membership fees are non-refundable. By subscribing to the Paramedic Program, the subscriber agrees to authorize the release of medical/insurance information for the purpose of emergency medical service billing only. Membership will begin upon receipt of payment and will not apply retroactively to any charges occurred prior to subscription.
